The process of recording and monitoring the vaccination against COVID-19 takes place through the national system for electronic records in health – My Term.
The following processes are included:
– Defining the target population for vaccination
– Vaccine data entry and management (warehouse)
– Vaccination scheduling
– Vaccination records
– Reports on vaccination dynamics and scope
The database for the targeted vaccination population contains the initial data for all persons who should be vaccinated, and is supplemented with data that will result from the vaccination process.
Data on the type and quantities of vaccines are entered in the national depot.
Scheduling of vaccination is done by the family doctor (first dose) and by the vaccination teams (second dose). The appointment is made in the national system My Term, in a precisely defined term. The appointment is made only for those persons who have confirmed interest in vaccination and have no contraindications.
Data entry in the individual records for each person is done by a member of the team from the vaccination point.
The following data are entered:
- Personal identity number of the patient
– Method of vaccination (I-first dose or II-second dose) and from the main warehouse at the national level, which monitors the situation with the quantities of vaccines.
The record for conducted vaccination becomes part of the electronic health file of the patient.
